For the old annual submission process, you need to use the older style re-certification form. Simply complete this form and send it to the CIPS office.
For the triennial submission process, you need to use the new style re-certification form. Consult the Re-certification Policies and Procedures Handbook on what you can claim as re-certification credits, how to earn and claim them, reporting requirements, as well as step-by-step instructions on how to renew your certification.
